Below are the various reports and forms for the Victim
Services Support (VSS) Program. You may download these forms and reports by
following the listed instructions. If you have any questions, you may contact
any of the VSS program staff listed below or call toll free at 1-800-373-5044.
Budget Revision Request Form:
Due to the fact that budget
line items are estimates of budget expenditures, funds may be reallocated among
budget line items. You can move amounts between the budget line items without
submitting a formal Budget Revision Request form if it is less than 10% of the
total award. If it is 10% or more, you must submit a completed Budget Revision
Request form and receive approval from VSS staff prior to requesting
reimbursement. Please email your completed Budget Revision Request forms to
your primary VSS contact or the Division Accountant. Our office will review
within 30 days and notify you regarding approval or denial. The Budget Revision
Request Form does not require a signature. Click on the following

Complete Program Budget Form:
The Complete Program Budget for
Fiscal Year 2009 (7/1/08-6/30/09) is due on September 1, 2008. Click on the

Fiscal Claim Vouchers & Spreadsheets:
Below are the Monthly &
Quarterly Claim Vouchers and Spreadsheets for Fiscal Year 2009
(7/1/08-6/30/09). This form must be completed and submitted on either a monthly
or quarterly basis. Attach documentation for expenditures claimed
for reimbursement or match. Please note that the forms are for all CVAD
administered funding streams. You may delete those sections from the
spreadsheet that do not apply.

Performance Reports:
Performance Reports are an
essential part of communication between programs and VSS Staff. These
required reports provide updates on the progress of the funded programs as well
as supply required statistical information. All VSS funded programs are
required to submit Performance Reports on a bi-annual basis. Below is a table
of the reporting periods and due dates.
